Information about Coronavirus (COVID-19)
Older adults and people with kidney disease or other severe chronic medical conditions seem to be at higher risk for more serious Coronavirus illness. Because of this increased risk for kidney patients, it is especially important for you to take actions to reduce your risk of exposure.

Request an Appointment
Due to COVID-19 we do not schedule same day appointments anymore. To request an appointment at Southwest Virginia Nephrology Medicine, PC call (540) 904-5366. Be sure to have your insurance information with you when you call. Our office will verify your insurance before the first visit. If there is a problem, we will contact you. If your insurance plan requires a referral from your primary care provider, we will let you know.

Please plan to arrive 30 minutes before your scheduled appointment time to complete paperwork. The New Patient Information Form can be downloaded (click here) and completed at your convenience at home or at our offices. We will ask you for demographic, contact and medical history information. You will sign a consent, acknowledgement and an authorization forms for treatment and release of personal health information. You will also be asked to review our Notice of Privacy Practices. On your first visit please be sure to bring:
- Insurance Card
- Driver’s License or Photo Identification
- Completed New Patient Information Form (click here)
- All Current Medications you are taking. Bring the medications rather than just a list of them.
Cancel an Appointment
If you must cancel a scheduled appointment, please notify us at least 24 hours in advance.
SWVA Nephrology Medicine hours: Monday thru Friday from 8am to 4:00pm
Closed daily from 12.00pm – 1.00pm
The office will be closed on the following days: New Year’s Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving Day, and Christmas Day.
We try to return calls as quickly as possible. If you leave a voice message for one of our clinical staff, your call will be returned by the end of the morning or by the end of the day. Calls received after 4 pm will be returned the following business day.
Lab Testing & Prescriptions
Lab Testing and Test Results
A physician may request that you have laboratory testing done before your scheduled appointment so that the test results are available to discuss with you at the time of your appointment. We do not have an in house lab and you will be directed to a draw station at Solstas Lab, LabCorp (Laboratory Corporation of America Holdings) or Pathgroup that is conveniently located for you. Results will either be mailed to you or you will be called.
To request a prescription refill from SWVA Nephrology, please call your Pharmacy and they will contact our office.
Insurance & Billing
We accept most insurance plans. Your insurance coverage will be verified when you make your first appointment. If there is a problem, we will contact you. We cooperate fully with patients’ insurance carrier; however, it is important that you are aware of any restrictions specific to your plan. Please call your insurance carrier if you have questions about coverage. We will do our best to help you.

Patients are responsible for paying deductibles and co-payments at the time of their visit. To make payment convenient, we accept all major credit cards, cash or checks.
